When you get to The Division 2 endgame (reaching 30 level and finishing main storyline accessing World Tier 1) all the equipment you own will be assigned a Gear score number - each gear piece, weapon and mod. These all contribute to your general Gear Score, which is used to measure your combat power. The concept is that you simply unlock better and improved equipment as you advance through the game.
Gear Score is a blend of two different variables: the power and quality (rarity) of the item. The rarity of the item is randomly determined when the loot drops: greater rarity equipment will naturally be better than those below them. Additionally, your Gear score determines the score for the loot that drops, but not from your current Gear Score. It consists from the highest possible gear score from all your equipment in your inventory along with your stash. It usually means that you don't need to wear your mathematically strongest gear all the time, only having that equipment on you or in you stash is enough for stronger loot to drop! However, your "effective" Gear Score is an average of all the items equipped on your character so just your equipped Gear Score affects your character's combat power level.
